【摘要】In order to obtain the best and continuous transition care from Intensive Care Unit to ward for ICU Severe Acute Pancreatitis patients, a nurse-led, multidisciplinary Transitional Care Model was established based on study on need of transitional care and qualitative research on shared decision-making experience for ICU Severe Acute Pancreatitis patients. Evaluation and care, communication and coordination, education and guidance were three main components of the model, standard discharge and liaison plan are needed for ICU SAP patients.%为了使重症胰腺炎患者从重症监护室(ICU)转到其他护理单元的过程中获得最佳的连续性的服务,通过对ICU延续护理服务内容需求的研究以及ICU重症胰腺炎患者家属决策参与体验的质性研究,构建了由ICU护士主导,其他部门医务人员以及患者和家属共同参与的ICU重症胰腺炎患者延续护理模式.包括评估与照护、沟通与协调、教育与指导三个主要指标以及建立标准化的ICU重症胰腺炎患者转科计划和联络随访计划.
