一、信息卡的正面与反面

二、信息卡的坐标线

图中画圈的地方是保证机器阅卷准确的坐标线，在任何时候不能有污迹和损伤，需要监考人员特别注意。在考试前打开试卷袋的时候，若发现坐标线有污迹和损伤，需及时更换，报考试院后使用备用袋，不得将坐标线有污迹和损伤的信息卡发给考生。

三、信息卡的内容及注意事项

为了便于讲解，我们将答题卡分成8个部分：

第一部分是学校和姓名栏目，由考生用签字笔填写。

第二部分是违纪和缺考栏目，由监考人员根据考场情况，在考试后期或收卷后用2B铅笔填涂。

第三部分是试卷代码栏目，由考生用2B铅笔填涂，这部分每次考试出错率非常高，考生要特别小心，认真填涂，监考人员在监考过程中和收卷时要认真检查，发现填涂错误要指出，并让考生更正。注意：考试级别已由程序设定，不需填写。

第四部分是准考证号栏目，空格部分由考生用签字笔填写，数字部分由考生用2B铅笔填涂，监考人员在监考过程中和收卷时要认真检查，发现填涂错误要指出，并让考生更正。

第一至第四部分需要注意的常见错误是：

1、考生准考号填涂错误；

2、a、b、c、d试卷代码不填涂；

3、违纪和缺考的答题卡不填涂或填涂不完整；（学校、姓名、缺考标记、试卷代码和准考证号等栏目全部要填涂）第五部分是是登分栏目，由阅卷老师填涂，考生不得填涂，否则作违纪处理。监考人员在监考时要认真检查，若发现考生填涂应当场制止并责令考生改正，拒不改正的作违纪处理。

第六部分是客观题的答题栏目，由考生用2B铅笔填涂，常见的错误见图示：

第七、八两部分是主观题的答题栏目，由考生用签字笔填写,需要提醒考生注意的是答题时不得写出红线，超出答题框限定区域（禁止在此区域答题）的答案无效,如下图黑色箭头标识部门的答案无效。
