狂犬病再次暴露疫苗接种程序探讨

[摘要] 狂犬病再次暴露后疫苗接种程序，相关规定不尽相同。该文分析了有关规定的相互关系，认为实际操作和《中国药典》不一致，存在隐患和法律风险;建议对《中国药典》和工作规范、技术指南相关条款及时修订更新，加快科学研究新成果的转化，并在法律层面加以规范，使狂犬病防控工作更科学、更有效。

Discussion on Vaccination Procedure for Rabies Re-exposure

[Abstract] The vaccination program after rabies re-exposure is not the same. This paper analyzes the relationship between the relevant regulations， and believes that the actual operation is inconsistent with the "Chinese Pharmacopoeia"， there are hidden dangers and legal risks; it is recommended to timely update and accelarate the relevant provisions of the Chinese Pharmacopoeia， work norms， and technical guidelines to accelerate the transformation of new scientific research results and regulate it at the legal level to make the prevention and control of rabies more scientific and effective.

狂犬病是严重威胁人民群众生命健康一种动物源性传染病，报告死亡数一直处在我国法定报告传染病前列。主要由狂犬病病毒通过破损的皮肤或黏膜侵入人体感染神经系统引起，临床大多表现为特异性恐水、恐风、进行性瘫痪、咽肌痉挛等。狂犬病暴露后规范处置是暴露后预防发病的唯一有效手段[1]。近年来，狂犬病发病虽然呈下降趋势，但由于其病死率几乎100%，暴露后的疫苗接种是预防狂犬病的核心措施之一，狂犬病疫苗的规范接种必须引起各级疾病预防控制机构和狂犬病暴露处置门诊的高度重视。笔者发现狂犬病再次暴露后疫苗接种程序，国家的有关规定不一致。实际操作中多数是依据《狂犬病暴露预防处置工作规范（2009年版）》第二十一条第二款的规定执行。例如2018年《北京市狂犬病暴露预防处置技术指南
