1. Middle Class Tax Relief And Simplification Act. An economic plan designed to grow the economy 4% per year and create at least 25 million new jobs through massive tax reduction and simplification, in combination with trade reform, regulatory relief, and lifting the restrictions on American energy.

Response Evaluation Criteria in Solid Tumors (RECIST) Quick Reference: Eligibility · Only patients with measurable disease at baseline should be included in protocols where objective tumor response is the primary endpoint. Measurable disease - the presence of at least one measurable lesion. If the measurable disease is restricted to a solitary lesion, its neoplastic nature should be confirmed by cytology/histology. Non-measurable lesions - all other lesions, including small lesions (longest diameter <20 mm with conventional techniques or <10 mm with spiral CT scan), i.e., bone lesions, leptomeningeal disease, ascites, pleural/pericardial effusion, inflammatory breast disease, lymphangitis cutis/pulmonis, cystic lesions, and also abdominal masses that are not confirmed and followed by imaging techniques; and. · All measurements should be taken and recorded in metric notation, using a ruler or calipers. All baseline evaluations should be performed as closely as possible to the beginning of treatment and never more than 4 weeks before the beginning of the treatment. · The same method of assessment and the same technique should be used to characterize each identified and reported lesion at baseline and during follow-up. · Clinical lesions will only be considered measurable when they are superficial (e.g., skin nodules and palpable lymph nodes). For the case of skin lesions, documentation by color photography, including a ruler to estimate the size of the lesion, is recommended. Methods of Measurement – · CT and MRI are the best currently available and reproducible methods to measure target lesions selected for response assessment. Conventional CT and MRI should be performed with cuts of 10 mm or less in slice thickness contiguously. Spiral CT should be performed using a 5 mm contiguous reconstruction algorithm. This applies to tumors of the chest, abdomen and pelvis. Head and neck tumors and those of extremities usually require specific protocols. · Lesions on chest X-ray are acceptable as measurable lesions when they are clearly defined and surrounded by aerated lung. However, CT is preferable. · When the primary endpoint of the study is objective response evaluation, ultrasound (US) should not be used to measure tumor lesions. It is, however, a possible alternative to clinical measurements of superficial palpable lymph nodes, subcutaneous lesions and thyroid nodules. US might also be useful to confirm the complete disappearance of superficial lesions usually assessed by clinical examination. · The utilization of endoscopy and laparoscopy for objective tumor evaluation has not yet been fully and widely validated. Their uses in this specific context require sophisticated equipment and a high level of expertise that may only be available in some centers. 2021年多学科诊断与治疗协作理念下胰腺癌外科治疗的进展(全文) 摘要 胰腺癌是高病死率的消化系统恶性肿瘤,手术技术的进步并未明显改善病人预后。随着综合治疗理念的普及,胰腺癌的外科治疗也由“surgery first”进入多学科诊断与治疗协作时代,在术前评估、新辅助治疗及微创技术的应用、术后辅助治疗等方面均取得较大进展。上述治疗的新理念、新方法可在一定程度上改善病人预后,但临床应用还存在争议。如何推动胰腺癌外科治疗向个体化、微创化、精准化方向发展仍是学术界的重要课题。笔者总结近年来多学科诊断与治疗协作理念下胰腺癌外科治疗的新进展,探讨目前其存在的局限和争议,针对胰腺癌外科治疗的未来发展提出思考,以期进一步促进其发展,改善病人预后。 关键词 胰腺肿瘤;治疗;外科;多学科;新进展 近年来,外科手术技术的进步以及靶向、免疫治疗等综合治疗手段的提升极大改变了肿瘤治疗的现状,但胰腺癌病人的预后仍无明显改善。Cancer Statistics 2020数据显示:胰腺癌5年生存率仅为9%,美国国立癌症研究所建立的监测、流行病学和最终结果数据库(SEER)中胰腺癌5年生存率为10%,较往年无明显提升,但发病率却逐年升高[1]。作为胰腺癌治疗的主要手段,外科治疗在过去的关注视角较

为局限,对胰腺癌缺乏全面认识。随着多学科团队(multiple disciplinary team,MDT)诊断与治疗协作理念的普及,胰腺癌外科治疗逐渐囊括了术前评估、新辅助治疗、微创手术及术后辅助治疗等多方面,通过MDT形成胰腺癌的全程管理模式。然而,胰腺癌外科治疗仍有许多争议亟待解决,笔者结合文献和实际经验对此进行总结,探讨胰腺癌外科治疗的未来发展模式。 一、胰腺癌的术前评估 胰腺癌可切除性评估是决定胰腺癌治疗方式的重要依据。胰腺CT 检查薄层扫描+三维重建已成为胰腺癌术前评估的首选方式,三维可视化技术也在胰腺癌术前评估中得到应用[2]。根据肿瘤与邻近血管的关系及有无远处转移,胰腺癌分为可切除、交界可切除、局部进展及转移性胰腺癌。然而,单纯的解剖学关系难以反映肿瘤真实生物学行为及预后。近年来,影像组学也开始应用于胰腺癌病人的预后评估。有研究结果显示:CT检查纹理特征与无进展生存时间和总体生存时间相关,能够较好预测病人预后[3]。一项纳入161例病人的研究利用CA19-9、术前病变的影像组学特征和Brennan评分建立预后预测模型,一致性指数(C-index)可达0.74[4]。但目前影像组学的研究仍处于起步阶段,样本量较少,需要更多临床研究进一步探索其效能。 二、胰腺癌的新辅助治疗
